Chronic Disease Self-Management Course (CDSMC)
Self-management is:
  • dealing with the consequences of the illness
  • concerned with problem solving, decision making and confidence building
  • a self-management course does not conflict with medical advice
  • patients and health professionals are in partnership

Brief History of the Chronic Disease Self-management Course
  • Kate Lorig – Stanford Model (1970’s)
  • Challenging Arthritis (1994)
  • Expanded in voluntary sector
  • Stanford  (Generic) Model put forward as an example of good practice
  • 1998 – White Paper ‘Saving lives: Our Healthier Nation’ proposes introducing the programme in NHS
  • 2000-Commitment to introduce Expert Patients Programme made in the NHS Plan
  • April 2002-staff with long-term conditions employed
  • Pilot period runs from 2002-2004
  • Aim is to have courses running in all PCTs by 2004

Who is the course for?
  • Adults of any age who have any long-term health condition
  • e.g. arthritis, asthma, back pain, cancer, diabetes, heart conditions, hormonal conditions, lung conditions, ME, MS, Parkinsons, stroke.
  •  People often have more than one condition

Why a Generic Course?
  • Patients with different conditions share many of the same problems
  • It is the consequences of the illness that are often more problematic than the disease itself
  • Social rather than Medical model

Volunteer Tutors
  • Success of the programme will largely be dependent on the successful recruitment and training of suitable volunteers
